Patients, families, and Providers are the center of the pediatric/adolescent medical home.

Together you and the medical home team manage your health and coordinate your care through additional services. As your medical home Carrow Street Pediatrics is responsible for coordinating your care across multiple settings and you the patient are encouraged to play an active role in your own healthcare

Carrow Street Pediatrics functions most effectively as a medical home when our patients and families provide complete medical history and information about care obtained outside the practice. We encourage you to update our staff at each visit.

At Carrow Street Pediatrics our physicians and nursing staff are committed to providing robust, comprehensive care that is thoroughly grounded in modern medical science. All of our diagnoses, treatments, and necessary referrals are made on a careful, evidential basis. We provide patient and family education and support at office visits, through anticipatory guidance and self-management materials on both our website and patient portal. 

Our Care Team provides evidence-based health care emphasizing the whole person including physical, emotional, behavioral, and mental health. This is achieved through mutually developed Treatment Care Plans by the Provider, patient and/or family. Behavior health is additionally supported through referrals to outside providers when deemed necessary.

Carrow Street provides equal access to all of our patients regardless of your source of payment. We are vigilant about maintaining the highest of care-giving standards. We view such standards to be not only the evidential approach but the ethical one as well.
